Health insurance after you move back to India
Your cover from abroad usually ends when you leave, and a hospital stay in a good Indian city hospital can cost lakhs. Here's what to buy, how much, and when.
The short version
How Indian health insurance works
- Policies pay for hospital stays, surgery and day-care treatment, usually cashless at network hospitals
- Most don't cover regular doctor visits or medicines outside hospital
- Premiums rise as you get older, and are paid every year
How much cover
- A base policy of ₹10–25 lakh for the family
- Plus a super top-up of ₹50 lakh to ₹1 crore, which is cheap for a big amount
- More if you live in a metro, where hospitals cost more
Buy early
- Buy soon after you land, or before you leave if an insurer allows it
- Existing illnesses are covered only after a waiting period of up to 3 years
- Each year you wait is a year longer before that cover starts
The parts of a good plan
| What it is | Why it matters | |
|---|---|---|
| Base policy | Your main cover, for the whole family or each person | Pays the first bills; choose one with no room-rent limit |
| Super top-up | Extra cover that starts after a set amount (the deductible) is used up in a year | A big cover for a small premium; set the deductible equal to your base cover |
| Parents' policy | A separate policy for parents | Keeps their higher premiums and claims from eating your family's cover |
| Employer cover | Group insurance from your job in India | Useful but ends when the job does; keep your own policy too |
Since 22 September 2025, individual health insurance premiums in India carry no GST.
What to check before you buy
- Waiting periodsUsually 30 days for most illnesses, up to 3 years for illnesses you already have, and often 2 years for some named treatments, such as cataract or hernia surgery.
- Room-rent limits and co-paymentLimits on room rent can cut your whole claim in proportion. Co-payment means you pay a share of every bill; common in senior plans.
- Network hospitalsCheck that the hospitals near you are in the insurer's cashless network.
- Restoration and no-claim bonusRestoration refills your cover if it runs out in a year; a no-claim bonus raises it each year you don't claim.
- Claim settlement recordLook at how many claims the insurer pays, and how fast.
- Be honest about your healthDeclare every condition. Leaving one out is a common reason claims are rejected.
Good to know
- Your cover abroad rarely carries over. Indian insurers usually don't count years of cover in another country, so waiting periods start fresh.
- There's no upper age limit any more. Since 2024 insurers must offer cover at any age, though premiums for people over 60 are high and often come with co-payment.
- Keep cover for trips abroad. Indian policies cover treatment in India; buy travel insurance for visits abroad, or a global plan if you'll travel often.
- Tax: premiums can be deducted only under the old tax regime (up to ₹25,000 a year for your family, ₹50,000 for parents over 60).

Questions
Can I buy Indian health insurance before I move back?
Some insurers sell to NRIs, sometimes with a medical check-up in India. Buying before you move starts your waiting periods sooner, so it's worth asking a few insurers.
How much health cover do I need in India?
For a family in a big city, a base policy of ₹10–25 lakh plus a super top-up of ₹50 lakh to ₹1 crore is a common choice. Treatment for a serious illness or a major surgery in a top hospital can cost ₹10 lakh or more.
Does my health insurance from abroad work in India?
Usually not, unless it's an international plan that names India. Local employer plans in the US, the Gulf, the UK and elsewhere end when you leave the job or the country.
Are existing illnesses covered?
Yes, after a waiting period of up to 3 years, set by the policy. Some insurers sell add-ons that shorten it for a higher premium.
Is a family floater or individual policies better?
A family floater is cheaper for a young family. Give parents their own policy: their age would raise the whole floater's premium.
